CPM Enhances CRM System for Population Health Management
CPM, a provider of healthcare customer relationship management (CRM) and strategic marketing solutions, announced its CRM system for population health management.
Leveraging the new enhancements that feature integration of quality metrics, hospitals can identify and act on opportunities to improve the health of their communities.
“With healthcare reform, Medicare reimbursements will be directly linked to better quality and lower costs. Our clients need a solution that allows them to easily manage the health of their population and report results,” said John Hallick, CEO & president of CPM, in a statement. “Our interface makes it easy to define a population, understand its health needs and then develop and execute strategies to change health behaviors.”
CPM's population health enhancements include tools for quality improvement and reporting, advanced targeting for population health management and improved performance metrics and reporting.
The company's CRM system includes HEDIS and PQRS quality measures approved by the National Center for Quality Assurance (NCQA) and the Centers for Medicaid and Medicare Services (CMS).
Users of the new CRM system can segment their patient population by measure and further segment by compliant or non-compliant patients. Using quality measures can improve patient outcomes, assist patients in assessing quality of care and drive payments and bonuses by private and public payers.
